As part of the annual oversight activities of the AIDS Institute’s HIV Quality of Care Program, organizations that provide medical care to people living with HIV (PLWH) in New York State (NYS) will be expected to complete the 2024 organizational HIV treatment cascade data submission template for care provided in 2023. This Excel template should be submitted to the New York State Department of Health (NYSDOH) AIDS Institute via the Health Commerce System; submissions that pass validation checks will be incorporated into a secure AIDS Institute database. The data submission template includes a worksheet for inputting patient-level data, sheets for visualizing cascade indicator results as charts and tables (these will be automatically generated from the provided patient-level data), and sheets for entering the organization’s QI tools, cascade methodology, key findings, and a quality improvement plan, including consumer involvement.
